At Community Health we provide quality, accessible, affordable primary care to everyone in our community.

What is primary care?​


Primary care considers a person’s overall health and well-being.

Your primary care provider will get to know you and your family’s health history and help you maintain a healthy lifestyle by identifying, tracking and controlling health risks.

Primary care includes general sick and wellness visits, monitoring chronic health conditions, and following up from hospital visits. It also includes:

  • chronic disease management
  • preventive health services and screenings
  • immunizations
  • cancer screenings
  • behavioral health, dental and specialist referrals
  • lab and blood work
  • X-rays
  • minor office procedures
  • sports physicals
  • workers comp injuries

Early identification and treatment during regular visits with your primary care provider is the best way to avoid future health complications.

How to make an appointment with a primary care provider​


You can make appointments with a Community Health provider by calling the Patient Access Center at 802-779-9169.

You also can request an appointment through our secure online Patient Portal.

Walk-in care for minor illness and injury is available at our Express Care locations in Rutland and Castleton.

Who are Community Health Primary Care Providers?​


Our family medicine doctors and advance practice clinicians are knowledgeable about the body and mind at all ages of life. They manage preventive health and chronic disease diagnosis and management for infants, children, adults, and seniors. Our pediatricians treat children and are experts in children’s physical and mental health needs.

Community Health’s advanced practice clinicians include certified nurse practitioners (NP) and physician assistants (PA). These highly-trained healthcare providers serve as primary care providers for most patient needs and work under a doctor’s supervision.

Primary Care Providers are your advocates​


At Community Health, primary care providers and their teams of healthcare specialists also act as your advocate. We help you understand the healthcare system through care management and community support.

Community Health providers work as a team delivering preventive services, care management, treatments, and educational support. Our goals include:

We take part in the assessment and delivery of care needed by the various demographics represented in our region. We also understand the impact of social factors of health (such as safe housing, transportation, discrimination, access to nutritious foods, and language skills) on general wellness, physical and behavioral.

Our goal is to build a relationship that lasts a lifetime, creating a comfortable and convenient patient-centered medical home (PCMH) for everyone.

What is a Patient-Centered Medical Home?​


Community Health is a recognized Patient-Centered Medical Home (PCMH) where health professionals work as a team to better manage your care and prevent potential health problems. We put the patient at the center of everything we do.

In a Patient-Centered Medical Home, your care team and your primary care provider know you and proactively work with you to anticipate your current and future health needs rather than have you wait for a future office visit to discuss them.

What is Care Management?​


Community Health’s Care Management team works alongside our primary care providers helping you manage your condition and medications while connecting you to the resources and support you need. Care Managers will help:

  • manage your health and medications
  • prevent unnecessary hospital stays
  • avoid trips to the emergency department
  • identify helpful community resources
  • assist with social barriers to healthcare like transportation, cost of prescriptions, food and housing insecurity

Medicare and Medicaid are accepted​


Community Health providers accept Medicare and Medicaid coverage, and our insurance navigators will assist patients with eligibility and coverage questions. We also are aware of special programs that are available to supplement primary care coverage.
